 Dynamic Healthcare at Home is a comprehensive, quality advanced, practiced-led healthcare service that provides Home Care/Caregiver Services, Primary/Transitional Care, Virtual/Telehealth and Medical Foot Care. Services are provided to individuals across the life span in the convenience of the individual’s home or residential community throughout Upstate, South Carolina. Our goal is to avoid potential hospital admissions by identifying health concerns and enabling earlier clinical intervention by collaborating with various post-acute entities to meet the needs of the community, while serving the health needs of individuals who would otherwise lack ready access to healthcare.
